Living Well With Chronic Conditions

The Crook, Deschutes and Jefferson County health departments are offering a six-week, online class to support people living with chronic health conditions. Learn from trained volunteer leaders with their own health conditions about how to:

* manage symptoms and medication
* work with your health care team
* set weekly goals
* problem-solve effectively
* improve communication
* relax and handle difficult emotions
* eat well and exercise safely

Set your own goals and make a step-by-step plan to improve your health—and your life. Each participant in the workshop receives a copy of the companion book, Living a Healthy Life With Chronic Conditions.

Classes are Wednesdays October 20 – November 24. $10 for the series (financial assistance may be available)https://www.yourhealthcentraloregon.org/workshops/
